Yes. Culture Connex provides personalised support to help individuals safely transition from hospital or rehabilitation back into the community. This may include returning to their own home, moving into Culture Connex disability housing (SDA, ILO, SIL, or MTA), accessing crisis accommodation, or connecting with other suitable housing options.

During the transition phase, we collaborate with the individual, their family, and clinical teams to ensure continuity of care and support. We can establish tailored support plans, including Culture Connex support workers and, where required, registered nursing services. Our goal is to create a safe, holistic, and person-centred environment that respects each person’s unique needs and promotes independence and wellbeing.
